Under-Eye PRF Filler
A rested under-eye, made from your own blood.
The under-eye is the one place we do not use hyaluronic-acid filler. Instead, Maria uses EZ Gel, a PRF filler made from your own blood: plasma is drawn, separated, and gently heated into a soft gel, then blended back with your platelet-rich fibrin and placed in the tear trough. It softens the hollow that casts the shadow while the growth factors work on the thin skin above it. Nothing synthetic goes into the thinnest skin on your face.

- Treatment time
- About 60 to 90 minutes, including the blood draw and preparing the gel
- Downtime
- Minimal. Expect some swelling for a few days and a chance of bruising, since the skin here is the thinnest on the face. Many patients plan it around a quieter week.
- Results
- Softer hollowing is visible right away, then swelling settles over the first week or two. Skin quality keeps improving over the following weeks as the growth factors work. Because it is your own tissue, the body gradually reabsorbs the gel, so most patients plan a short series and then periodic maintenance, which Maria maps out at your consult.
What under-eye filler actually does.
The under-eye shadow that bothers you is usually not pigment, it is topography. As the midface loses volume, the ligament that tethers the skin along the orbital rim becomes a visible groove, and the groove casts a shadow in overhead light. Soften the groove and the shadow has nothing to form against. The question is what you put there, and in the thinnest skin on the face Maria's answer is your own tissue rather than a synthetic gel.
EZ Gel starts with a routine blood draw. Your plasma is separated in a centrifuge, then gently heated so its proteins set into a soft, smooth gel, and blended with liquid platelet-rich fibrin before it is placed in the tear trough with a blunt cannula. The gel gives the hollow back some cushion; the PRF delivers growth factors that thicken and brighten the skin over it. That second effect is why patients often notice the under-eye looks less tired as well as less hollow, and it is something a traditional filler does not do.
Hyaluronic acid attracts water, and under the eye that can show up as puffiness or a bluish cast through thin skin. Those are the reasons Maria does not place hyaluronic-acid filler here at all. EZ Gel avoids both, and because it is made from you, there is no foreign material and no allergy question. The trade-off is that the body gradually reabsorbs it, so it is a series and maintenance treatment rather than a single long-lasting syringe. Most patients find that a fair exchange for a result that looks and feels like their own skin.
Under-eye shadows come in a few different forms, and matching the plan to yours is what produces a result you love. Hollowing with resilient skin responds beautifully to the gel. Thin, crepey skin responds to the PRF itself, often as PRF skin therapy injections or alongside Morpheus8 to firm at depth. Darkness that comes from pigment responds to skin treatments and sun protection. And sometimes the most striking change comes from the cheek above, where restoring support lifts the whole area, which is a facial balancing conversation. Patients come to us for under-eye work from New Haven, Hamden, Branford, and Guilford.
- A hollow or groove under the eye that casts a shadow
- Looking tired in photographs regardless of how much you slept
- Thin, crepey under-eye skin that shows the darkness beneath
- Patients who want under-eye volume without synthetic filler
- Tear trough (inner under-eye)
- Mid and outer under-eye hollowing
- Transition between under-eye and cheek
Adults with under-eye hollowing or thin, tired-looking under-eye skin, including patients who would rather not have synthetic product in the area. Not suitable during pregnancy or breastfeeding. Under-eye concerns come in several forms, so the consult starts by working out which one you have: if morning puffiness, loose skin, or pigment is part of your picture, we build that into the plan alongside or instead of the gel.
Swelling and bruising are the common ones and settle within days to about a week. There is a small bruise from the blood draw as well. Because the gel is made from your own blood, allergic reactions are not a concern, and the bluish cast that hyaluronic acid can cause in thin under-eye skin is not a risk with PRF.
Sleep slightly elevated for the first two nights, which helps the swelling settle faster. No pressure, rubbing, or makeup on the area for 24 hours. Skip heavy exercise, heat, and alcohol for 48 hours. Arnica if you bruise easily. Call us with any question rather than massaging the area yourself.
